ACKNOWLEDGMENT OF RISK, WAIVER, AND RELEASE OF LIABILITY
You will be participating in numerous exercise sessions and other similarly physical activities as a member of Maverick Training Center. You have the option of seeking medical advice regarding these matters BEFORE participating in this program.
THIS DOCUMENT IS A RELEASE OF CLAIMS AND BY SIGNING IT, YOU RELINQUISH CERTAIN LEGAL RIGHTS.
I assert that any and all participation by me in physical activity during this training, whether in training sessions, on the property, or elsewhere on and off the grounds, will be voluntary. In consideration for being allowed to participate in this training, I, on behalf of myself, my spouse, my heirs, and my estate, hereby release, waive, indemnify, and save harmless Maverick Training Center, and all Maverick Training Center instructors, employees, volunteers, or agents from any and all liability from and all physical or other injuries or harm (including these resulting from the ordinary negligence of Maverick Training Center, its employees, volunteers, or agents) suffered by me during, or as a consequence of, participation in any and all physical activity associated with training.
I further understand that there are serious inherent risks involved with all physical activity and that when performing exercise routines or engaging in martial arts or boxing or any similarly strenuous activity, I may suffer minor injury, serious injury, or even death. I understand these inherent risks, and I assume all such inherent risks associated with participation in any activity are in conjunction with the training and release Maverick Training Center, all instructors, employees, volunteers, or agents from liability.
Additionally, I understand that the activities offered at Maverick Training Center involve close contact with other people and may result in an increased risk of exposure to and/or contraction of infectious diseases including but not limited to COVID-19. By signing this agreement, I acknowledge and voluntarily agree to assume the risk of exposure to such contagious diseases while participating in activities at Maverick Training Center.
I further represent to Maverick Training Center that I am in excellent health and physical condition and that I am not disabled in any way, taking medication, or suffering from any condition that would prevent me from safely engaging in such activities or that would make such participation potentially dangerous or harmful for me.
I hereby waive, release, and discharge any and all claims of damages for death, personal injury, or property damage which I may have, or which may hereafter accrue to me, as a result of my participation in the training. The release is intended to discharge in advance Maverick Training Center, instructors, employees, volunteers, and agents from and against any and all liability arising out of or connected in any way with my participation in the training, even though that liability may arise out of negligence or carelessness on the part of the person or entities mentioned above who (through negligence, carelessness, or otherwise) might be liable to me or my heirs or assign, for damages. It is further understood and agreed this release and assumption of risks is to be binding on my heirs and assigns.
